The Su-30MKM is a variant of the Su-30MKI fighter jet designed for the Malaysian Air Force. In August 2003, Malaysia signed a US$900 million contract for the delivery of 18 Su-30MKM fighter jets. Russia and India jointly carried out the contract, with India manufacturing the wings, stabilizers, and tail, and Russia manufacturing the remaining parts. As part of the agreement, Russia was also supposed to send Malaysia's first cosmonaut to the International Space Station. Delivery of the fighter jets began in 2007 and was completed in 2009. On November 27, 2016, the Su-30MKM successfully dropped a GBU-12 Paveway laser-guided bomb during exercises. This made it the first Russian-made combat aircraft compatible with this weapon. However, by 2018, 14 of the 18 aircraft had been grounded due to engine issues. A local company recieved 2 billion ringgit for their modernization, and the program was completed in 2025. Currently, Su-30MKM fighters remain on combat duty with the Malaysian Air Force.

The Malaysian Su-30MKM was introduced in Update 2.53 "Line of Contact", as a part of the Japanese aviation research tree. It is nearly identical to the Su-30SM from the USSR tree. It has engines with thrust vectoring, which significantly increases maneuverability at low speeds. Its weaponry consists of RVV-SD missiles with a fairly long range. Unlike its Soviet counterpart, the aircraft has a DAMOCLES container with a thermal imager. The MKM differs from the Russian version in that it has MAW and LWS, increasing the aircraft's protection against missile attacks. One of the aircraft's shortcomings is its heavy weight, which negatively affects its acceleration and maneuver capabilities, especially with its low-thrust engines.
